Immunization State law requires that you submit documented evidence from a physician that your child has received the following immunizations:

Diphtheria Toxoid, Tetanus Toxoid, and Pertussis Vaccine (DPT) -- Every child should have received a minimum of four doses, one of which should have been given on or after the fourth birthday, or any five doses. If TD is given, a doctor's note must accompany the immunization report.

Oral Polio Vaccine -- Every child should have received any four doses.

Measles (Rubeola) Virus Vaccine -- Every child should have received two doses (or two MMR doses) administered on or after the first birthday.

Mumps Virus Vaccine -- Every child should have received one dose (or two MMR doses) administered on or after the first birthday.

 Rubella Virus Vaccine -- Every child should have received one dose (or two MMR doses) administered on or after the first birthday.

Hepatitis B Vaccine -- Every child should have received three doses.

Varicella -- Every child must have received this vaccine or have proof of having the chicken pox.

Mantoux tuberculin test required as directed by the Dept of Health-speak to the school nurse regarding this requirement as it pertains to your child.
